What happens if the doctor can't issue a fit to model letter?

Our doctors will base their letter on their assessment of you during your appointment. They may ask you to change aspects of your lifestyle before coming back to them after a certain amount of time eg 2 months.

Do I need to bring anything to my appointment for a fit to model letter?

Please bring any information you have about any pre-existing conditions that you have.
