Frequently asked questions

Can I receive my event form as a paper copy signed and stamped?

Yes, if you need a hard copy, your doctor will sign and stamp it during the appointment. An electronic copy will also be available in your DocTap account.

If I am over 40, do I need an ECG and Cholesterol blood test?

Patients aged 40 and above at the time of their appointment are required to undergo an ECG and a cholesterol blood test for an event medical. If you have results of these from a health screen done in the past 2 years we can use these results instead.

If you can't fill out my form, can you advise where I can get this done?

Your NHS GP is generally the best place to start for more complex medical forms and letters. We do not have access to your medical history and this is often required for employment,university or other complex medical letters.

Will I receive my signed medical form the same day?

Your form will be available shortly after your appointment. However, if it requires blood test results or other investigations, it will be completed as soon as those results are received.

Can my form be completed on a video consultation?

As the majority of medical forms require a physical examination, a video appointment may not be appropriate and the patient will need to be seen in person. If you have already been seen in one of our clinics we may be able to complete your form in a video appointment. Please email your form to support@doctap.co.uk asking whether it can be completed via video appointment.

How do I know if you are able to complete an event medical form for me?

We can complete most simple event medical forms provided there is no requirement to access your medical history. If you are unsure about whether we can complete your event medical form please email it to us at support@doctap.co.uk so we can check whether our doctors can complete it.

I have an event in Italy, can you complete my form?

b. Spirometry (a lung function test) Italian event medical forms require us to have evidence of a Resting ECG, Stress ECG and spirometry. We do not provide stress ECGs or spirometries at DocTap. If you are able to bring in evidence of having done these with a different provider our doctors can fill in the rest of your Italian event medical form. If a sporting event is in Italy, for a patients certificate to be signed they will need to bring in results for the following tests undertaken within the last 12 months: Patients forms cannot be signed without these tests as this is according to Italian law for medical sports certification. We unfortunately cannot complete these test for patients as we don not have accsess to a Spirometry machine, or a stress ECG provider. a. Resting ECG and a Stress ECG

My form is not in English, can a doctor still fill this out if I translate it for them?

We are unable to fill out a form that is not in English even if you translate it for us. Please contact the event organisers to obtain an English version of your application form.

Will my event form be available electronically?

Yes, please ask your doctor to upload your form to your notes. It will be available in the notes section of your DocTap account.
